Doggie Day Care Agreement ......
There are two sides/pages. Form must be filled out completely. Sign and initial in all required spaces. If you need assistance, please call Jaime at 760-207-5267. Please bring in the original - we will take a copy to give you for your records.
Kamp Kanine Walk-In Agreement .......
All registrants must complete this form regardless of whether they will be using our walk-in service. Customers needs may change from time-to-time so we require this form be on-file.
Pick-up & Drop-off instructions .......
All registrants must complete this form. In the event of an emergency, traffic issues or miscommunication, we will need to have alternative drop-off instructions.
Credit Card Authorization Form ......
All registrants must complete this form. For emergency vet treatment or delinquent account issues, we must have this form on-file. It is file and stored securely and not accessible to anyone except management.
Current Vaccinations Records .....
This form should be taken to your vet for completion and given to us before your dogs first day at kamp.
On 3/15/2010 we implemented a monthly maintenance fee of $25.00 for those accounts that were missing any of our required documents. Many clients fail to supply Kamp Kanine with proof of current vaccinations or a copy of the certification of valid dog license with the county. To avoid this monthly charge, please make sure you turn in all documents we have requested. Please feel free to call us if you need to know what vaccinations are due to expire. We appreciate you helping us keep your account current.

Kamp Kanine Fees and Payment Policies -
Day Care Rates (Effective 04/1/2010)
- Fees below are calculated and charged at a per dog,
per day rate.

- Clients purchasing pre-paid discount packages by credit card will NOT incur this fee but agree to recurring billing and their card will be charged when package is down to 5 or fewer uses.
- Daily Dog rate is for dogs that attend 5 days a week, every week.
- Multi Dog rate is only for dogs in the same household.

If paying by check, and not purchasing a package,
services must be paid for in advance or on day of service.
